New Patient Registration
To register as a new patient, you will need to live within our practice boundary.

You can download the three registration forms below and send back to us at:
Reading University Health Centre, 9-11 Northcourt Avenue, Reading RG2 7HE.
Please include a copy of your I.D and proof of address e.g passport, photo driving licence and a utility bill or tenancy agreement if you have them. You do not need proof of address or immigration status, ID or an NHS number to register unless you require online access but it might help if you have one or more of the following:
- passport
- birth certificate
- HC2 certificate
- rough sleepers' identity badge
- hostel or accommodation registration or mail forwarding letter
If you're homeless, you can give a temporary address, such as a friend's address, a day centre or the GP surgery address.

If you are under 16 years of age please download the three forms below for completion. We will require a copy of the vaccination history to be provided for all children registering with a GP in England for the first time. If you are unable to provide a copy, we can take a verbal history to establish whether there are any outstanding vaccinations for the UK schedule and an appointment will need to be arranged with a nurse to discuss this.

2 Come in
Come in to our reception and ask to register. Reception is open Monday to Friday 8am – 6.30pm.
Please bring with you a copy of your I.D and proof of address e.g. passport, photo driving licence and a utility bill or tenancy agreement if you have them.
What Happens Next?
We will enter your details onto our computer system, and for patients who have previously registered with a doctor in the UK arrange for transfer of your medical records to us.
If you need to see a doctor or nurse you can book an appointment from one week after we have received your completed forms. This is to allow us time to process your registration.
If you take regular medication for contraception, asthma, hay fever, acne or eczema you will need to book an appointment with one of our specialist nurses at least a week before your medication runs out. (Please tell reception which condition your medication is for so they can direct you to the correct nurse).
All other medications please book an appointment with a doctor at least one week before your medication runs out.

Our Healthcare Assistants are trained to undertake New Patient Health Checks.
